2017-2018 September DC/TL Timeline

What We Do
  1. Complete the Monthly Special Education Roster/SHARS Documentation google doc - follow instructions outlined in the Monthly Medicaid Roster procedures - Due September 8 (date adjusted due to storm) - This will replace the Special Education Team Roster.
  2. Meet with SpEd team at least twice to distribute information from DC/TL meeting and to support team in: maintaining student information, developing draft PLAAFP statements, Goals & Objectives, Accommodations, understanding & completing Form 2, understanding Service Logs, etc.
  3. Monitor the use of the Parent Communication Log when initial contact has been made with parents (identify those teachers who have not introduced themselves to the parents and communicate the district expectations. 
  4. Organize folders for maintaining student service logs: each student should have a manila folder to maintain service logs for the year.  This folder should remain in locked file cabinets organized alphabetically (many DC/TLs keep this information alongside the campus IEP folders - this system will keep paperwork organized for End Of Year check-in.  
  5. Ensure that the ARD Teacher Input Forms is being distributed to all staff (GenEd & SpEd) at least 3 weeks prior to the ARD/IEP meetings (completion due date is 2 wks prior to the annual ARD meeting). 
  6. Ensure teachers are completing and have sent to parents the draft PLAAFP, Goals & Objectives, and accommodations at least 5 days prior to the ARD/IEP meeting.
  7. Attend SpEd DC/TL Connection Meeting September 26 - Online Mtg. 
  8. Secondary DCs: Ensure progress reports for IEP Goals/Objectives have been issued to all students who receive Special Education (End of the 6 week period is Sept. 22 & report cards go home on Sept. 29).
  9. Complete 15 Day Count and submit to Campus Coordinator before 2:30pm on September 15
Item to be reviewed by September 12, 2017:
  • Parent Communication Log showing 2 contacts: 1) BOY Parent Letter distribution date 2) Initial phone call to connect with parents including highlights of the conversation
Items to be collected and submitted to Coordinator by September 15, 2017:
  1. Signed Verification of Services Form including each student that you case manage.
  2. Signed BOY Spreadsheet
  3. Original Verification of Receipt of Accommodations and Modifications including signatures from all teachers who teach each student
  4. Student roster 
  5. Paraprofessional Region 16 Certificate of Completion
  6. 15 Day Count
