- Introduce yourself to the students you case manage (this should be done discretely & completed before September 2, 2016).
- Let students know how to reach you if they need you outside of the course you are instructing. Let them know that you will maintain constant contact with them to make sure they are getting what they need to understand what’s being taught (you may want to set a weekly schedule or predetermined way to check-in with them in a discrete manner).
- Discuss with teachers your plan to ensure students have what they need to stay on course with progress toward their IEP goals and objectives.
- Speak with Special Education teachers who provide instructional services to the students that you case manage and let them know when and how you will be collecting their student service logs.
- Distribute & explain the importance of the ARD Teacher Input Forms to all staff who instruct any students who have ARD/IEP meetings scheduled in October (put a completion due date 2 wks prior to ARD).
- Complete draft PLAAFP, Goals & Objectives, and accommodations at least 5 days prior to the ARD/IEP meeting.
- Request a sample of student work with documented Accommodations & talk to teachers regarding accommodations for the individuals (to be kept in student binder).
- Attend SpEd Connection Meeting.
- Secondary Case Managers: Distribute IEP Goals/Objectives progress report to be issued concurrently with the report cards (End of the 6 week period is Sept. 23).
Items to be completed and placed in teacher binder or submitted to DC/TL by September 2, 2016: - Have parents/students sign that they received your welcome letter and that they know how to reach you, support they should expect to be available in specific classes, and how you plan to monitor their specially designed instruction each week. Put signature indicating receipt of information in your binder (DC/TL will check on Sept 2, 2016 & report % of students who have signed the acknowledgement form – District expectation 100%).
- File a copy of the Verification of Receipt of Accommodations and Modifications form with signature in the student's IEP folder on campus (retain a copy in your binder) originals will be submitted to the state folder by DC/TL. Signatures indicated that teachers have received and understand the student’s PLAAFP statement, accommodations, BIP, goals and objectives (as they may affect various classes outside of the specified content area – ie reading affects all course disciplines), and ARD Committee deliberations – (District expectation 100% by September 2).
- Retrieve all student service logs for the month of August (Aug 22-Sept 2), analyze services to ensure minutes have been met, discuss and give the monthly analysis of student service log to your DC with the analysis of services.
