2016-2017 March Case Manager Timeline

What's Required
  1. Attend monthly Connection Meeting. 
  2. Ensure students have and use their accommodations for STAAR Grade 4 and Grade 5 (March 28-30).
  3. Continuously review the Verification of Services to make sure student services match their Schedule of Services and they are receiving the full minutes.
  4. Ensure S.T.E.P. Binders are complete and in the room of each general education teacher.
  5. Check with Special Education and General Education teachers who instruct your students to find out how the program is working for the student & if there are any concerns/questions regarding the level of support that the student is receiving. 
  6. Collaborate with general education teachers regarding the implementation of InSync and monitor the use of InSync. 
  7. Ensure the completion of Student Profiles with core general education teachers for all special education students grades 3-8 and for high school EOC testing areas. 
  8. Distribute Teacher Input Forms to all staff who instruct any student who has an ARD scheduled in March (due date – 2 wks prior to ARD). 
  9. Retrieve Teacher Input Forms from all teachers who have completed them for ARDs this month. 
  10. Connect with all parents who you have an ARD meeting scheduled with this month to discuss ideas for the new plan that will be finalized during the ARD meeting (let parents know that you will be sending the draft PLAAFP statement and goals/objectives home to be reviewed). 
  11. Update communication log and collaborate with teachers any information from parent communication that is pertinent to their class/supports they should be providing.   Make weekly contact with teachers of students on your caseload and document in the student’s log whether the students are on task with passing grades, class assignments, homework, attendance and behavior.  
  12. Retrieve the Service Logs for the students you case manage and review services being provided. Discuss discrepancies with teachers providing the service. If service concerns have not been addressed, review concerns with campus SpEd DC/TL. 
  13. Talk to parents. Make a weekly phone call to parents of students on your caseload and edit in your call log.  Be sure to communicate any information from your contact with their teachers on all concerns or areas where the student is off task (passing grades, class assignments, homework, attendance, and behavior).  Find out if there’s anything that they need to come home that can help them with their child’s learning. Find out if they are getting timely communication from their child's teachers or if there is any other way that you can assist them (being proactive goes a long way).  Congratulate parents on something good the student is doing.
  14. Meet with the students that you case manage at least twice outside of the class that you teach them in. Find out what they need help with. Recommend a book that they should read. Find out if there is any area that they are having difficulty and develop a plan with them to address the concerns.
Items to be completed during, or prior to the end of, March: 
  1. Elementary Case Managers: Distribute IEP Progress reports (end of nine weeks Mar. 21) & maintain verification of distribution - possible options for distribution: 
    1. Place a copy with the student's teacher who distributes report cards and have it stapled behind the report card going home. 
    2. Send progress report home with the student. 
    3. Send progress report home via mail (discuss postage with campus administrator). 
  2. Draft PLAAFP, Goals/Objectives, Accommodations and send to parents (as a draft) 5 days prior to scheduled IEP/ARD meeting. 
  3. Make sure accommodations/supplemental aids are prepared/available for students to receive before the March 28-30 STAAR for 4th grade Writing, 5th grade Reading, 5th grade Math, 7th grade Writing, 8th grade Math, and 8th grade Reading.
  4. Ensure the completion of Student Profiles with general education EOC tested areas. 
  5. Establish meeting dates for Form 2 review with general education and special education teachers of all 5th graders and 8th graders on your caseload.  Use current data and student progress to refine the needs of each student.  The FORM 2 will be required for each 5th and 8th grader by April 14th.

Items to be completed by April 4: 
  1. Retrieve all Student Service Logs for the month of March (March 6-31), analyze services to ensure minutes have been met, discuss and give the monthly analysis of student service log to your DC with the analysis of services. Highlight any concerns and note if they are recurring concerns that need administrative assistance. 
  2. Meet with DC/TL to discuss student data on benchmarks and to discuss the data collected on the effectiveness of the students' supports/accommodations. 
  3. File Student Service Logs and student work samples for the month of March in a folder designated for the student that is kept in a secure/confidential location (locked file cabinet).
