2019-2020 April Case Manager Timeline

What's Required

Items to be completed April 3:
  1. Post all completed sessions in MSB xLogs up to March 5th to indicate services provided. 
  2. Prepare to provide services and instructional supports beginning April 6 for Seniors and April 13 for all other grade levels.
Items to be completed & submitted to DC/TL prior to April 15:
  • Promotional Staffing Prep: Current Schedule portion of the Promotional Staffing Sheet for all 5th and 8th grade students that you case manage.
  • Review of all IEP components for Case Manage students to determine appropriate services/supports for accessing instruction and making progress on Goals and Objectives and with the Project Based Learning. Consider: What can reasonably be done to support the student during COVID 19 school closures?
Begin April 6 for Seniors (and those beyond their Senior year) & April 13 for all other grade levels:
  • Collect data on progress towards mastery of Goals and Objectives
April Actions
  1. Attend monthly Connection Meeting.
  2. Use the Case Manager Guidance document to guide your thinking, and decision making to ensure the students who you case manage receive appropriate services during 
  3. Communicate with all related service and instructional personnel who have provided services to the students you case manage to determine services during this interim period of remote Project Based Learning. 
  4. Communicate with parents/guardians of students you case manage regarding the level of support that will be provided during remote learning PBL. Include services outlined in the Schedule of Services page. If Parents are in agreement, complete IEP Amendment (according to training) to document interim changes during school closure remote learning and to state that all services will resume when we return to face-to-face instruction.
  5. Check in at a minimum of 2 times/week with students/parents of students who you case manage to support their Project Based Learning. Support students with accessing the PBL, organizing their learning time, submitting assignments, and answering any questions they may have.
  6. Follow guidance of campus principal regarding participation in grade level/content area PLCs.
  7. Check with General Education teachers who are grading the students you case manage (as appropriate) to find out how the student is progressing through the Project Based Learning and if they are submitting assignments.
  8. Grade student work according to Rubrics designed to identify students who may need a different level of support.
  9. Document the provision of accommodations using the Accommodations Log or MSB xLogs.
  10. Send Teacher Input Forms to all staff who instruct any student who has an ARD scheduled in April (due date – 2 wks prior to ARD).
  11. Retrieve Teacher Input Forms from all teachers who have completed them for ARDs this month and use this information to develop PLAAFP statements.
  12. Connect with all parents who you have an annual ARD meeting scheduled with this month to discuss ideas for the new plan that will be finalized during the ARD meeting (let parents know that you will be sending the draft PLAAFP statement and goals/objectives home to be reviewed). 
  13. Update communication log (or eSped Parent Contact Log) and collaborate with teachers/related service provider(s) any information from parent communication that is pertinent to their class/supports/services they should be providing. 
  14. Provide all instructional services as outlined for the students who you case manage (academic, behavior, social).
  15. Talk to parents. Find out if there’s anything that they need to help them with their child’s learning. 
  16. Meet with the students (and/or parents) that you case manage at least twice a week (more if providing direct special education as outlined on the SOS page). 
Items to be completed prior to April 24:
  1. IEP Amendment ARDs will be held to identify interim supports for remote Project Based Learning (inclusive of instruction supporting IEP Goals/Objectives, Direct Special Education Services, and Instructional Services based on students individual needs).
  2.  Secondary Campuses: update and archive Progress Report on IEP goals/objectives beginning February 24 - April 17:
Unacceptable responses: "in progress" or "minimal progress"
Acceptable responses: 4 out of 5 opportunities, attends to task for 15 minutes, 80% accuracy

Items to be completed by May 1:
  • Ensure all services for students you case manage are documented using MSB xLogs (Senior Teachers - beginning 4/6/20 all other grade levels 4/13/20).
