Services Documentation (MSB X Logs)

What's Required

Case law has demonstrated time and time again that “If it isn’t written down, it wasn’t done." School staff and districts can each be held financially responsible for lost services and the burden of proof for service documentation is on the service provider. 

What We Do

All special education staff who provide a direct or indirect special education service on behalf of a student are required to document the details of the services provided. 

Examples of typical direct services: 
    • Co-teaching
    • In-Class Support
    • Specially Designed Pullout 
    • Counseling (either group or individual)
    • Therapy (OT, PT, Speech, Music)
    • Instructional Services (APE, VI, O&M, Bilingual)
    • PASS/SILC Monitoring
    • Testing Students
Examples of typical in-direct services: 
    • Attending an ARD Meeting
    • Coaching a teacher regarding the implementation of the IEP/BIP (Monitoring with Facilitated Support)
    • How to use Assistive Technology
    • Writing Assessment Reports
Starting in the 2018-2019 school year, all documentation of services are to be documented in MSB Xlogs. Please see details below regarding how to document the services. 

Xlogs accounts are tied to the specific provider. Under no circumstances should a provider share their login information with another provider. Additionally, services documented must be the services provided by the specific provider as this is legal documentation regarding ARD'd services.

Co-Teach  / In-Class Support Documentation Expectations

Co-Teach/ In-Class Support
  1. Set up reoccurring schedules in Xlogs in advance
  2. On the day the service is  provided
    •  Write a Narrative regarding what services were provided (use the session Narrative if it applies to all people or individual student Narrative if applies to specific student only)
    • Mark if the student was present or absent from the session you provided services (the student could be present at school but absent from the session)
    • Check off the accommodations/modifications/supports provided for each student
    • Use the tickers to identify how many times the accommodations/modifications were provided
    • Update goals and objectives (not required to be done in Xlogs for the 2019-2020 school year)
  3. At the end of the day or no later than the last day of the week - update the session details and Post session

CASE Program / AB Program Documentation Expectations

Teacher
1. Setup reoccurring schedules in Xlogs in advance for ALL your students (whole day for all students - may include time from bus arrives to depart if ARD'd)
2. On the day the service is  provided
      • Write a narrative regarding what services were provided (use the session narrative if it applies to all people or individual student narrative if applies to specific student only)
      • Mark if the student was present or absent from the session you provided services (the student could be present at school but absent from the session)
      • Check off the accommodations provided for each student
      • Use the tickers to identify how many times the accommodations/modifications were provided
      • Update goals and objectives (not required to be done in Xlogs for the 2019-2020 school year)
3. At the end of the day or no later than the lasts day of the week - update the session details and Post session

Paraprofessional
1. Setup reoccurring schedules in Xlogs in advance for the students you monitor
2. On the day the service is  provided
  • Write a Rationale as to why the student was taken to another room outside of the program classroom (ex. Attended Art Class in GenEd)
  • Write a Narrative regarding what services were provided (use the session Narrative if it applies to all people or individual student narrative if applies to specific student only) 
  • Mark if the student was present or absent from the session you provided services (the student could be present at school but absent from the session)
  • Check off the accommodations/modifications/supports provided for each student
  • Use the tickers to identify how many times the accommodations/modifications were provided
3. At the end of the day or no later than the lasts day of the week - update the session details and Post session

PASS Program / SILC Program

Teacher

3. On the day the service is  provided
      • Write a Narrative regarding what services were provided (use the session narrative if it applies to all people or individual student Narrative if applies to specific student only)
      • Mark if the student was present or absent from the session you provided services (the student could be present at school but absent from the session)
      • If the student is pulled, add a new non-reoccurring session to document the student being pulled
        • Write a Rationale as to why student was pulled
        • Write a Narrative of the services provided (ex. Social Skills Lesson)
        • Check off the accommodations/modifications/supports provided for each student
        • Use the tickers to identify how many times the accommodations/modifications were provided
        • Update goals and objectives (not required to be done in Xlogs for the 2019-2020 school year)
4. At the end of the day or no later than the lasts day of the week - update the session details and Post session

Paraprofessional

Related Service Staff (Homebound, OT, PT, APE, Bilingual, VI, O&M, Counseling)

1. Setup reoccurring schedules in Xlogs in advance for all your students with a reoccuring sessions
2. Write a Rationale for the service you are providing and where it occurs (ex. in CASE Classroom, Pullout Time, etc.)
3. On the day the service is  provided
      • Write a Narrative regarding what services were provided (use the session narrative if it applies to all people or individual student Narrative if applies to specific student only)
      • Mark if the student was present or absent from the session you provided services (the student could be present at school but absent from the session)
      • Check off the accommodations/modifications/supports provided for each student
      • Use the tickers to identify how many times the accommodations/modifications were provided
      • Update goals and objectives (not required to be done in Xlogs for the 2019-2020 school year)
4. At the end of the day or no later than the lasts day of the week - update the session details and post session

Behavior Specialists, & Program Specialists

Behavior Specialists & Program Specialists

1.  Most events will be entered on the day of service. However, if you have reoccurring schedules enter them in Xlogs in advance for all your students with a reoccuring sessions
2. On the day the service is  provided
      • Write a Rationale for the service you are providing and where it occurs (ex.  Modeling student's BIP for the AB teacher in the AB Classroom)
      • Write a Narrative regarding what services were provided (ex. guided the teacher on how to use deescalation technique)
      • Mark if the student was present or absent from the session you provided services (the student could be present at school but absent from the session)
      • Check off the accommodations/modifications/supports provided for each student
      • Use the tickers to identify how many times the accommodations/modifications were provided
3. At the end of the day or no later than the lasts day of the week - update the session details and post session

Diagnosticians & LSSPs

Diagnosticians & LSSPs

1. Most events will be entered on the day of service. However, if you have reoccurring schedules enter them in Xlogs in advance for all your students with a reoccurring sessions. Most common sessions types will be "Assessment - Testing",  "Assessment - Report Writing", and "ARD Meeting".
2. On the day the service is  provided
      • Write a Rationale for the service you are providing and where it occurs (ex.  pulled student from CASE Classroom to perform testing for the student's re-evaluation)
      • Write a Narrative regarding what services were provided (ex. completed rating scale)
      • Mark if the student was present or absent from the session you provided services (the student could be present at school but absent from the session)
      • Check off the accommodations/modifications/supports provided for each student
      • Use the tickers to identify how many times the accommodations/modifications were provided
3. At the end of the day or no later than the lasts day of the week - update the session details and post session

Documentation for Retired Goals & Objectives (2020-2021 and beyond)
Providers are able to "activate" old Goals and Objectives for 12 hours, if they are needing to document on ones that have expired (retired), providers can use the following steps if needed. 

1.Navigate to the Student Menu section of the Caseload Column by clicking on the student’s name you want to access.

2.Select Goals and Objectives.

3.Locate the goal in question within the list of goals. 4.Click on the goal to load it in the workspace.

If the goal dates are incorrect in X Logs, they can be manually changed to reflect the current date so the provider can document. Remember, the Goals and Objectives are updated nightly during our integration process, so it will change back to the original date.
