Incidents/Accidents - STUDENT - For Incidents or Accidents that occur during the courses of the school day and involve students participating in VICTORY or CBVI
If student require emergency medical attention
- Call 911
- Contact the Campus Administrator, School Nurse, and the Director of Special Education Accountability & Related Services
- Ensure an Administrator (Campus, Transition Coordinator, or the Director of Special Education Accountability & Related Services) remains with the child during transport and at the hospital until parents arrive
- Contact the parent(s) and provide general information regarding the incident, status of their child, and hospital.
- Complete Special Education Incident Report Form , the Student Injury Report,
- Scan a copy of the Injury report to the Secretary to the Executive Director of Special Education and the Director of Special Education Accountability & Related Services
- Take steps to minimize the risk of a similar incident/accident from occurring again.
If student DOES NOT require emergency medical attention
- Contact the Campus Administrator, School Nurse, and the Director of Special Education Accountability & Related Services
- Receive input from the nurse regarding next steps
- Contact the parent(s) and provide general information regarding the incident, status of their child, and hospital.
- Complete the Special Education Incident Report Form and the Student Injury Report
- Scan a copy of the Injury report to the Secretary to the Executive Director of Special Education and the Director of Special Education Accountability & Related Services
- Take steps to minimize the risk of a similar incident/accident from occurring again.
Incidents/Accidents - STAFF/CONTRACTORS - For Incidents or Accidents that occur for STAFF/CONTRACTORS that report to the ADMIN-Special Education Department
If a staff member requires emergency medical attention
- Call 911
- Contact the Campus Administrator, Nurse, staff member's supervisor, and the Director of Special Education to which the staff member reports
- Ensure an Administrator (Campus, SPED Coordinator, or the Director of Special Education for which the staff member reports) remains with the staff member/contractor during transport and at the hospital until emergency contact arrives
- Contact the staff member's emergency contact and provide general information regarding the incident, status of the individual, and hospital. (Contact the Secretary to the Executive Director of Special Education for Emergency Contact Info)
- Complete Special Education Incident Report Form, the Staff Member’s First Report of Injury Report , and the Supervisor Accident Investigation Report Form
- Scan a copy of the Special Education Incident Report Form, the Staff Member’s First Report of Injury Report , and the Supervisor Accident Investigation Report Form to the Secretary to the Executive Director of Special Education and the Director of Special Education to which the staff member reports
- Take steps to minimize the risk of a similar incident/accident from occurring again.
If student's DOES NOT require emergency medical attention
Worker’s Compensation Process- Employee is hurt on District property.
- Employee reports to the campus nurse, safety officer, or their supervisor
- The Supervisor contacts the Campus Administrator, Nurse, and the Director of Special Education to which the staff member reports
- Employee completes the Staff Member’s First Report of Injury Report
- Note: The Staff Member’s First Report of Injury report must be completed by the employee immediately, even if they are not seeking medical treatment or reporting loss of time.
- Employee provides the completed form to their supervisor for signature.
- Supervisor Completes Special Education Incident Report Form and the Supervisor Accident Investigation Report Form
- Supervisor scans a copy of the Special Education Incident Report Form, the Staff Member’s First Report of Injury Report and the Supervisor Accident Investigation Report Form to the Secretary to the Executive Director of Special Education and the Director of Special Education to which the employee reports
- The Secretary to the Executive Director of Special Education emails the worker’s comp email address.
- Employee receives an email from the Workers’ Comp Specialist acknowledging receipt of the paperwork with the employee’s steps attached.
- Employee will receive another email from the Workers’ Comp Specialist detailing the following information
- Employee Name
- Employee ID#
- Claim # for the employee to report to a physician
- The Worker’s Compensation Coordinator’s contact information
- Employee will report to the approved physicians who accept worker’s compensation claims. A list of approved facilities are located on the Human Resources website under the workers’ comp tab.
- The physician will send the work status report to Workers’ Comp.
- The employee has the responsibility to provide a copy of the medical release/documentation from their treating physician to Workers’ Comp.
- If the employee has been provided medical documentation to return to work with restrictions, the employee should contact the Workers’ Comp Specialist via email and/or phone immediately.
- If the employee has payroll deducted benefits and they are not receiving a Spring ISD paycheck, theymust make arrangements to pay for insurance premiums with the Spring ISD Payroll department. Please call the Payroll department at 281-891-6095.
- Workers’ Compensation rules instruct insurance companies to pay the first week of temporary income benefits after the second week of absence (7 days, does not have to be consecutive). Employees may elect to use their sick and/or personal days in lieu of workers’ compensation income benefits.
- Workers’ Compensation generally pays 70% of an employee’s average weekly wage up to a maximum set by the state of Texas. The maximum is changed annually by the Division of Workers’ Compensation of TDI. Employees who earn less than $10.00 per hour receive 75% of their average weekly wage. - Dates of absence and return to work dates affect your claim and your paychecks. When released from the physician to return to work, the employee must provide a copy of the Division of Worker’s Compensation WorkStatus Report, DWC-73. A determination will be made by the Human Resources Team as to whether or not the employee are able to perform the job responsibilities associated with their position.
Incidents/Accidents - VISITOR - For Incidents or Accidents that occur for VISITORS that reported to the Southridge Center, ESY, or on CBVI Trip
If a VISITOR requires emergency medical attention
- Call 911
- Contact the Campus Administrator, Nurse and the Director of Special Education Accountability & Related Services
- Ensure an Administrator (Campus, SPED Coordinator, or the Director of Special Education Accountability & Related Services) remains with the visitor during transport and at the hospital until their emergency contact arrives
- Complete Special Education Incident Report Form and the Visitor Injury Report
- Scan a copy of the both Special Education Incident Report Form and the Visitor Injury Report to the Secretary to the Executive Director of Special Education and the Director of Special Education Accountability & Related Services
- Take steps to minimize the risk of a similar incident/accident from occurring again.
If VISITOR DOES NOT require emergency medical attention
- Contact the Campus Administrator, Nurse, staff member's supervisor, and the Director of Special Education Accountability & Related Services
- Receive input from the nurse regarding regarding recommendations. If the visitor feels they do not require a doctor visit they may waive going to a doctor.
- Complete Special Education Incident Report Form and the Visitor Injury Report
- Scan a copy of the both Special Education Incident Report Form and the Visitor Injury Report to the Secretary to the Executive Director of Special Education and the Director of Special Education Accountability & Related Services
- Take steps to minimize the risk of a similar incident/accident from occurring again.
